NEW YORK (Reuters Health) ? A new study suggests men with advanced prostate cancer may live longer in the ?PSA era? than they did before the screening test began to gain a foothold in the early 1990s.
The findings ? which a leading cancer expert said were problematic ? stoke the heated debate over prostate cancer screening using the prostate-specific antigen (PSA) test. Many researchers now fear routine testing may lead to aggressive treatments that do more harm than good.
For the new study, published in the Journal of Urology, researchers tapped into data from three clinical trials involving more than 3,000 men who got hormone treatment for prostate cancer that had spread to the bones and other body parts.
The average patient survived 30 months after starting treatment in 1985 to 1987. By 1989 to 1994, survival was three months longer and by 1995 to 2009, well into the ?PSA era,? it had reached 49 months.
?It?s a remarkable difference,? said Dr. Ian Thompson, who heads the Cancer Therapy and Research Center at the University of Texas Health Science Center at San Antonio and led the study.
?The translation of this is, might it very well be that for advanced prostate cancer finding it earlier may allow the earlier initiation of therapy that may then reduce the death rates from the disease?? he told Reuters Health.
But the chief medical officer of the American Cancer Society, who was not involved in the work, said the study has a number of serious shortcomings.
For instance, other trials done by the same research group but involving non-Hodgkin lymphoma, a group of blood cancers, also show patients live longer over time, according to Dr. Otis Brawley.
And that?s despite the facts that doctors don?t screen for the disease and that patients got the same treatment for their disease.
?There is a cohort effect that we have seen numerous times in medicine,? said Brawley, who has collaborated with Thompson on earlier research.
He added that the reasons for the improved survival over time are unclear, but could be related to advances in medical care and infection control.
It is also possible that men today are more aware of symptoms that would have been ignored a quarter-century ago, such as trouble urinating, Brawley said.
?Awareness of the disease increases the chances that you are going to be diagnosed with it and that you will do better,? said Brawley.
That, along with better healthcare, may also help explain why the survival gap between black and white men narrowed over time in the new study, he added.
DIFFERENCE DIFFICULT TO EXPLAIN
Finding a tumor due to symptoms is different from finding it through screening, which per definition is done in symptom-free people.
When people have symptoms, their disease is typically more serious, and so people diagnosed with cancer as a result of screening automatically live longer with their disease than those diagnosed based on symptoms ? whether or not they get treatment.
This phenomenon, known as lead-time bias, is another limitation of the new study, which Thompson acknowledged.
He said that even after accounting for factors that are tied to earlier death in prostate cancer ? such as extensive disease, bone pain and high Gleason scores (a grading system) ? men in the later trial still survived longer.
?You cannot say that PSA testing is the cause for this improvement, but when you eliminate all the other potential explanations there is something else there that is playing a role,? he said.
But eliminating those other differences through statistics ? in other words, trying to cancel out the differences in cancer risk and stage among the men ? would also wash out any potential effect of screening. That?s because the point of screening is to catch the cancer as early as possible.
Acknowledging that, Thompson said, ?I guess I have a difficult time explaining the difference? in survival.

Is the cost of doing business poised to skyrocket on January 1? With tax cuts about to expire and spending cuts poised to take effect, the small business community seems to think so. The WSJ?s Washington Wire blog has the story:
A group that lobbies for small businesses released a big survey showing that uncertainty over the economy and fiscal policy have zoomed to the top of the list of businesspeople?s concerns. Health-care costs and taxes remain big bugaboos too, according to the once-every-four-years survey by the National Federation of Independent Business.
For the past four years, the Obama administration has been reassuring small businesses that his policies are designed with them in mind, and that marquee initiatives like Obamacare and Dodd/Frank will make life easier for them. But a rapidly shifting regulatory burden, the most turbulent economy in a generation, and an atmosphere of uncertainty in Washington have left business owners unimpressed.
This is bad news for Obama heading into November. Without small business enthusiasm, we won?t see a true employment boom.

The central part of Romney?s strategy is allowing states to be responsible for the permitting process for energy development and drilling on federal lands within their borders. Romney claimed his plan would add three million new jobs, reduce the trade deficit by 80 percent, and lure companies to escape more expensive energy prices abroad to build manufacturing plants in America again. This is a direct appeal to the blue-collar Reagan Democrats Romney needs to beat Obama.?
Romney noted that in North Dakota, it takes 10 days to get a permit for a new well. In Colorado, Romney noted that it took 27 days. In contrast, it takes an average of 307 days to get the same permit when it comes to federal lands.?
By giving states the power to regulate the permitting process as a part of his broader energy strategy, Romney said his plan would ensure Americans ?won?t be buying oil from Middle East or Venezuela if we don't want to.?
Romney said ?energy independence is the key to American leadership? and denounced Obama for taking federal dollars and picking losers by giving government-guaranteed loans and grants to green boondoggles like the failed Solyndra.
?The government of the United States is not a good venture capitalist,? Romney said, noting that his energy strategy would focus on investing in science and research that would find technologies to make energy use more efficient instead of gambling on crony capitalism.
Romney also said the federal government should stop ?using the law to stop the production of energy? offshore and would work with the Carolinas, Virginia, and the gulf states to find more targeted areas for offshore energy development while also making sure there are state-of-the-art safety procedures to prevent another BP oil spill.?
Romney also called for a partnership with Canada and Mexico, the Keystone pipeline to be built, and a more accurate inventory of the country?s energy resources.
Romney said Obama's claims about how much of the world?s energy supply America has is ??a dramatic understatement? and Romney would seek to get a more ?accurate inventory? of the country?s domestic energy supply.?

ST. LOUIS (AP) ? The nation's most withering drought in decades only got worse in several key farming states last week, despite cooler temperatures that at least gave those living there a break from this summer's stifling heat, according to a new drought report released Thursday.
In its weekly map, The U.S. Drought Monitor showed that as of Tuesday, just over two-thirds of Iowa, the nation's biggest corn producer, was in extreme or exceptional drought ? the worst two classifications. That's up more than 5 percentage points, to 67.5 percent, from the previous week.
Nearly all of Nebraska, Kansas, Missouri and Illinois are in extreme or exceptional drought, with Illinois showing the most-dramatic climb in those categories, spiking 17 percentage points in one week, to 96.72 percent, according to the map. In neighboring Indiana, where 5 inches of rain fell in some parts, the area of the state in exceptional or extreme drought fell 9 percentage points, to 37.09 percent.
Conditions cooled in the region, but little or no meaningful rain fell, said Mike Brewer, a National Climatic Data Center scientist who put together the latest weekly U.S. Drought Monitor map released by the National Drought Mitigation Center at the University of Nebraska in Lincoln. The lack of rain allowed exceptional and extreme drought conditions to continue to expand in the area from northern Missouri and into Kansas and Nebraska, he said.
Plains farmers have begun harvesting what corn managed to survive this summer, although in some areas growers cut their fields weeks ago, chalking the year up as a loss. Many ranchers have sold livestock because they had no grass for grazing or money to buy feed, which has soared in cost.
In the lower 48 states, there was a slight increase in the overall area experiencing at least some drought, from 61.77 percent on last week's map to 63.2 percent on this week's. There was little change in the overall area experiencing exceptional or extreme drought, which went from 6.26 percent last week to 6.31 percent this week.
Rain is expected in the northern Plains in the next few days, although it may come too late to save the region's withered crops.
The U.S. Agriculture Department twice has slashed its forecast for this year's corn and soybean output because of the drought in the nation's breadbasket. It forecast the nation's biggest harvest ever in the spring, when farmers planted 96.4 million acres of corn ? the most since 1937. But the agency has cut its estimate twice since then and now expects the nation to produce 10.8 billion bushels, the least since 2006.
If that estimate holds, the federal government says it will be enough to meet the world's needs and ensure there are no shortages. But experts say food prices will almost certainly climb as corn is a widely used ingredient in products ranging from cosmetics to cereal, colas and candy bars.
