In Florida, Obama calls for heat on Congress

President Barack Obama continued his economics road show on Thursday, coming to the Jacksonville Port Authority to underscore his commitment to infrastructure investment and bash Republicans for delaying progress.

The president chose Jacksonville because the port is home to two port development projects that he fast tracked last year as Congress remained mired in a seven-year delay over an authorization bill.

With his shirt sleeves rolled up, Obama told the crowd of about 500 port employees, union members and Democratic VIPs that as ?Washington keeps taking its eye off the ball,? with ?political posturing and phony scandals,?? he is prepared to continue to use his executive powers to advance projects if Congress won?t act.

?I?m not going to let gridlock or inaction or indifference to the plight of families get in the way of this country,?? he said in the 26-minute speech inside the port?s steamy passenger terminal. ?So where I could act on my own, I?ll act on my own.??

The president?s criticism was at times broad, saying that too often ?Washington hasn?t just ignored the problem, a lot of times it?s made things worse.? But he reserved his harshest swipes for the Republican-controlled House which has resisted his appeals on the federal debt limit and mandatory budget cuts.

?Shutting down the government just because I?m for keeping it open, that?s not an economic plan,?? he said. ?Threatening that you won?t pay the bills in this country when we?ve already racked up those bills, that?s not an economic plan. That?s just being a deadbeat.?

As an example, Obama referred to Congress? failure to pass a water resource bill that would provide the authorization to allow the Jacksonville port to deepen the Mile Point channel it says it needs to allow supertankers, which can hold the load of other cargo ships, to come from Asia through the Panama Canal.

Bringing in more supertankers ?works for everybody,?? he said, because it means ?more contractors are getting jobs,? have more money ?to spend at the restaurant,? and that brings more money for the waitress ?to buy her iPod.?

He noted that manufacturing jobs are the cornerstone of the middle class, as are investments in education, science and research, but ?too many folks in Washington have been cutting these investments.?

The president?s road show drew the scorn of the Republican Party of Florida at a news conference before the president?s arrival.

?It?s time for the president to lead,?? said state Sen. John Thrasher, R-Jacksonville, a former Republican Party chairman. ?He?s been president for 5 1/2 years and what he?s basically doing is back to the same old campaign mode and that?s not leadership.?

Gov. Rick Scott, who was out of state attending a meeting of the Republican Governor?s Association, sent out a statement bashing the federal corporate tax policy.

?High taxes hurt job creation, plain and simple,?? Scott said. ?If the president wants to turn around our national economy as we have turned around the economy in Florida, he should follow Florida?s lead and cut the federal corporate income tax so more manufacturers and businesses can invest and create jobs.??

The president?s speech is part of a series of addresses scheduled over the next several months intended to call attention to the plight of the middle class and outline his ideas for reform.

He sounded the same themes and cribbed many of the same lines from the two speeches he gave Wednesday in Illinois in Missouri, emphasizing the need to build the country ?from the middle class up? and vowing to devote the remainder of his term ? ?1,266 days? ? to the goal.

But as the president attempts to shift the debate in Washington back to his campaign narrative of the middle class, he offered few specifics. He also emphasized that he will listen to ideas from anyone.

?I don?t claim to have a monopoly on every good idea,?? he said to laughter. ?I?m happy to steal good ideas from anybody, because I just want to make things work.?

The president attacks on Congress come at a time when both his and Congress? popularity rating are at a two-year low.

A McClatchy-Marist poll last week found Obama?s job approval at 41 percent, his worst showing in the poll since 39 percent in September 2011, while public approval of congressional Republicans, who control the House of Representatives, was at a dismal 22 percent. An NBC/Wall Street Journal poll also conducted last week found that 83 percent of Americans disapprove of the job Congress is doing.

Pope brings in lay experts to help reform Vatican

VATICAN CITY (Reuters) - Pope Francis has brought in international experts to help him overhaul the Holy See and move on from a series of scandals under his predecessor Benedict XVI.

Urged on by the cardinals who elected him four months ago, Francis set up a commission on Friday to help him reform the Vatican's administration and finances.

Made up of seven international lay experts and one cleric, the commission will report directly to the pope and advise him on economic affairs, improving transparency and enforcing accounting principles.

Its members will have the right to examine any paper and digital document in the Vatican.

Francis inherited a Church struggling to deal with priests' sexual abuse of children, alleged corruption and infighting in the central administration and conflict over the running of the Vatican's scandal-ridden bank.

Benedict left a secret report for Francis on the problems in the administration, which came to light when sensitive documents were stolen from the pope's desk and leaked by his butler in what became known as the "Vatileaks" scandal.

The documents accused Vatican insiders of corrupt dealings, including the award of contracts to outside companies at inflated prices.

The new commission's lay members are experts in economics, finance, management and law and come from Spain, Germany, Italy, Singapore, Malta and France, the Vatican said in a statement. The cleric will act as the commission's secretary.

It will draft reforms of the Holy See's institutions to simplify how they work and improve the way they manage their finances.

They will advise Vatican departments and find ways to "avoid the misuse of economic resources, to improve transparency in the processes of purchasing goods and services", the statement said.

BROAD POWERS

Francis ordered all Vatican departments to collaborate with the commission and bypass usual rules that oblige officials to respect the secrecy of their office.

In reporting directly to the pope, the commission, which Francis set up with a personal decree known as a "chirografo", will bypass the Vatican's Secretariat of State, its chief administrative office which itself has been hit by allegations of scandal and corruption.

The new commission was Francis' third bold move to reform badly tarnished Vatican institutions.

One month after his election, he set up an advisory board of cardinals from around the world to help him govern the Catholic Church and reform its administration.

Last month, in an attempt to get to grips with an institution that has embarrassed the Catholic Church for decades, he set up a special commission of inquiry to reform the scandal-plagued Vatican bank.

The new commission will coordinate its work with the advisory board of cardinals and hold its first meeting after Francis returns to Rome after his July 22-29 trip to Brazil.

Harassment: Redmond police responded to a disturbance at an apartment complex in the 8600 block of 164th Avenue Northeast on Education Hill at 1:32 p.m.

Theft: Redmond police investigated a theft of an iPad at 9:32 a.m. from the 2200 block of 173rd Avenue Northeast. The victim reported his adult son stole the iPad and traded it in for cash at a video game store.

Vehicle prowl: Redmond police responded to and investigated a car prowl at 8:42 a.m. from a residence in the 4100 block of 169th Court Northeast in Overlake.

Neglect and abuse was present at the time i had my elderly father admitted as a resident in the Park view care center nursing home located in Paris Texas.
"Just imagine", if you where sick with some thing and you continued to get worse as the days went by how long would you stick it out before you went to see the doctor 2 days , 3days, 4days, 5 days , . The first month my elderly father was a resident in ParkView Care Center owned or run by [Diversicare management Services home office located in Tennessee] he went 23 days before one of the day nurse's decide he needed to be taken to the hospital.
I had been harping to nurse's about my elderly fathers condition every day before he was sent to hospital by ambulance[ I WAS IGNORED] .
To make a true and factual statement about 3 of the nursing homes i had my elderly father in ,i noticed one thing in common with all of the three nursing homes . The staff are friendly to family members when they are on the sight of the nursing home. If a family member makes a special effort like being at the nursing home every day 7 days a week for extended periods of time. The staff and the employees become paranoid about your presents, reason they get paranoid is they become lax and complacent in their duties and tend to set around and do nothing when ever they get the chance.

The following statement happened to me at ParkView Care Center located in Paris Texas. I had a female nurse make special effort to confront me for about three days in a row when i walked into to check on my elderly father, this nurse would walk up to me as i entered the ParkView Care Center nursing home and she ask me --(DONT YOU HAVE A JOB YOU NEED TO GO TOO ) this nurse was i assume trying to intimidate me i guess .
I was offended by this statement but said nothing at the time . The red headed female nurse did this the following 2 days as i went to visit my elderly father.
This kinda of behavior leads me to think that they [ OF COURSE I'M SURE THEY WOULDN'T ADMIT TO THIS THOUGH ] ' where trying to get me to stop my regular visits. [ GOD FOR BID I WOULDN'T WON'T TO INTERFERE WITH THE FOLKS CHAIR AND RELAXATION TIME, IF YOU CATCH MY DRIFT. ]
I myself have seen bed alarms at PARK VIEW CARE CENTER NURSING HOME go off beeping for 2 to 3 hours at a time from the elderly residence rooms ,which needed a nurse to check on them .The third shift was the worse! On regular occasion's the residence bed alarms would be ignored by the nurse's at the nurse's station for periods from 1hour up to 3 hours at a time. [ Good God where is the compassion , these people should be ashamed of their selves.] ,One night i was up late with my elderly father watching television in the front of the building .
I heard the night nurse scream at the top of her voice at one of the elderly female residence's. The night nurse jumped up from her chair [ setting down imagine that ] and trotted from behind the nurse's station the whole time screaming at the top of her voice at the elderly woman , as the nurse got close enough to the chair which the elderly woman set in , the nurse grabbed both arms of the elderly residents chair and the nurse placed her face about 2 inches from the elderly resident face screaming as loud as she could at the elderly resident, for spitting on the floor. The poor old lady had Parkinson's .
The elderly woman didn't realize where she was, the elderly lady was handicapped. The night nurse must have lost her mind . [scared the holy hell out of the old lady]. The nurse did not know i was up front with my elderly father watching television late in the evening when this event happened. Once the night nurse noticed me watching what she was doing , she quickly walked back to her chair behind her official nurse's station. The night nurse scared the old lady so bad the elderly woman did not say anything verbally out loud to anyone the whole time that i noticed while i was visiting at this nursing home and i was there 7 days a week 5 to 8 hours a day.

One thing i will not forget is the bed alarms which go off at night from the elderly residents rooms . The nurse's on the night shift set at the nurse's station and ignore the loud beeping alarms and the flashing lights. [good god this is pitiful] this was a regular thing on the late shift for the most part .
The most upsetting thing i happen to witness once was an elderly resident go code blue [ stop breathing ] one night . The young nurse on duty ran from the room to the phone to call the doctor on the phone [she said the doctor would not answer the page] then she gave up on trying to reach the doctor she then called her head nurse at home [this all happened about 9:45pm at night] .
Mean while the elderly resident has gone with out breathing for about 9 or 10 minutes at this point, the head nurse told the young nurse on duty to call 911 and get the EMS guys to try to revive him . She then called 911 the EMS guys took about 6 minutes to arrive . Point of this story is the elderly man coded blue and went about 14 too 15 minutes with out any attempt to revive him ,while i stood watching all this [I thought to myself good god is this the way you do things] 14 too 15 minutes is way too dam long to let an elderly person go before you attempt to revive him.
[ This code blue happened at LEGENDS NURSING HOME in Paris Texas ]. The things which go on in nursing homes is ungodly i could write a small book but i will keep the rest of this to my self for now.

I WOULD LIKE TO SAY THIS TO ANYONE READING THIS, IF YOU HAVE FAMILY THAT YOU TRULY LOVE AND ARE CONCERNED ABOUT ,"DO NOT" FULLY TRUST STRANGERS TO TAKE CARE OF YOUR LOVED ONES . One last note PARK VIEW CARE CENTER IN Paris Texas should be closed down . One more thing if your loved one dies while in the care of the Park view care center be prepared for a complete change in the way the staff at this facility talk with you and treat you if you return to discuss pro rated billing [ which means they try to stiff you with back dated charges] or to visit other residents at this nursing home .
The staff becomes real real paranoid and unfriendly . I even had the janitor walk up to me and with a condescending smile get in my face and he said [ don't make me have to throw you out of here] ""WHAT A DUM ASS THIS GUY WAS"".
I was always polite with all employees at this nursing home in fear that my elderly father might be treated badly in retaliation if i had made someone mad. I will post more about the Paris Texas Nursing homes in my area in coming articles . I WRITE THESE ARTICLES FROM MY EYE WITNESS FOR THE BENEFIT OF THE ELDERLY AND THE HANDICAPPED PEOPLE, WHO NEED ALL THE HELP THEY CAN GET BECAUSE THE ELDERLY FOR THE MOST PART ARE HELPLESS AND NEED THE YOUNGER PEOPLE TO GET THERE ASS IN GEAR AND HELP OUT NOT LOOK FOR A EASY WAY OUT .

(July 18, 2013) Most professional athletes are able to return to competition within a year after vertebral fusion surgery on the upper (cervical) spine, reports a study in the July issue of Neurosurgery, official journal of the Congress of Neurological Surgeons. The journal is published by Lippincott Williams & Wilkins, a part of Wolters Kluwer Health.

Another study in the July Neurosurgery finds variations in treatment for patients with minor head injuries seen in the emergency department (ED) versus the doctor's office. A third paper discusses the increasing importance of philanthropic funding for neurosurgery research and program development.

When Can Pro Athletes Return to Play after Cervical Spinal Fusion?

Dr. Joseph C. Maroon of University of Pittsburgh Medical Center and colleagues analyzed the treatment and outcomes of one type of cervical spinal fusion surgery, called anterior cervical discectomy and fusion (ACDF), in 15 professional athletes. The study focused on criteria for making return-to-play (RTP) recommendations.

The patients were eight football players and seven wrestlers. All were cleared for RTP after they had a normal neurological examination and x-rays showing vertebral fusion. Thirteen players returned to their sport within two to twelve monthsthe average RTP time was six months. Eight athletes are still participating; five retired after competing for another one to three years.

"After a single-level ACDF, an athlete may return to contact sports if there are normal findings on a neurological examination, full range of neck movement, and arthrodesis," Dr. Maroon and coauthors write. They note that athletes undergoing ACDF may be at risk for future spinal degeneration or disease, one level above or below the vertebral fusion.

Variations in Treatment for Minor Head Injuries

Dr. Rebekah Mannix of Children's Hospital Boston and colleagues analyzed national data on more than 10 million health care visits for minor head injuries between 2005 and 2009. These included about 6 million visits to EDs and 4 million to "outpatient" settingsdoctors' offices or hospital-based clinics.

Advanced imaging tests (CT or MRI scans) were more often performed for patients seen at EDs: 63 percent, compared to 14 for those seen at doctors' office and 21 percent at hospital clinics. A return visit was arranged for 54 percent of patients at doctors' offices and 29 percent at hospital clinics. Arrangements for follow-up care were made for 67 percent of patients seen at EDs.

Although the study shows variations in care, it permits no conclusions about which type of care is best. Dr. Mannix and coauthors call for further studies of natural recovery after minor head injuries, and how outcomes are affected by different treatment strategies.

Philanthropy for Neurosurgery Research Funding

In a special article, Dr. Edie E. Zusman of Sutter East Bay Neuroscience Institute, Castro Valley, Calif., and colleagues discuss the evolving role of philanthropic funding for neurosurgery research and program development. Such activities have traditionally been funded by government and industrybut as these sources decline, philanthropy has become "an increasingly important mechanism for building, maintaining, and expanding neurosurgical research programs."

The authors discuss some creative approaches to fundraising for research, such as "venture philanthropy" or forming nonprofit foundations and partnerships. In addition to "grateful patients," Dr. Zusman and coauthors conclude, "[C]ollaborations with health organizations, foundations, venture capitalists, and industry can also help to build the research, training, and clinical programs necessary to improve patients' lives."

Watchdogs warn of U.S. healthcare insurance exchange delays

WASHINGTON (Reuters) - Two U.S. government watchdogs warned on Wednesday of possible delays in the scheduled October 1 start of new state healthcare exchanges, raising further doubts about the implementation of President Barack Obama's signature legislation.

One watchdog warned the Internal Revenue Service would have difficulty completing testing of systems required to implement the law, while another from the Government Accountability Office said he also foresaw difficulties in meeting an October 1 deadline.

But Obama administration officials offered assurances to a congressional committee that they were on track. Acting Internal Revenue Service chief Danny Werfel told the committee the tax agency was "on target." Separately, Mark Iwry, a health policy senior adviser at the Treasury Department, also told lawmakers the administration's healthcare work was proceeding on time.

The administration officials' remarks came amid worrisome comments by watchdogs including Alan Duncan, an auditor with the Treasury Inspector General for Tax Administration, an arm of the IRS that monitors the agency's performance and readiness.

Duncan said the IRS work that must be done to test the systems "will be difficult to complete," by the October 1 start date.

"The lack of adequate testing could result in significant delays and errors in accepting and processing...applications for health insurance coverage," he told the House of Representatives Oversight and Government Reform committee.

At the same hearing, Government Accountability Office official John Dicken said he also saw problems implementing the healthcare law. The GAO is the investigative arm of Congress.

Beginning in October, individuals will be able to buy health insurance through the new exchanges in order to comply with the Affordable Care Act (ACA).

The exchanges are essential to the healthcare law's "individual mandate," which requires people to have insurance or pay a fine. These exchanges will extend coverage to millions of uninsured Americans by offering subsidized insurance through online marketplaces in all 50 states.

Many states have refused to set up the exchanges, adding to the federal government's burden in implementing ACA, which begins for individuals next year. The federal government has to set up exchanges for individuals in 34 states.

The U.S. Treasury and White House said earlier this month that businesses would not be required to offer health coverage, or pay a fine, in 2014 because the administration had failed to issue final regulations in a timely manner.

Congressional Republicans, who have taken more than three dozen votes to repeal the law since it was signed in 2010, are using the delay to argue the entire law is flawed. Another House vote is expected later on Wednesday.

The votes have little chance of moving in the Democratic-controlled Senate.

The total work that the federal government needs to do in each state is still to be determined, raising the risk of missing deadlines, GAO's Dicken said, adding the federal government and states have already missed some exchange deadlines.
